To register, visit the Contact Us page on the University of Chicago Celiac Disease Center website, choose "Blood Screening Registration" as the "Reason" for contact, and include the following in your email:
You are eligible for blood screening if you have:
- Been on a gluten-containing diet for at least 12 weeks prior to testing
- Close family member that has celiac disease or Type 1 diabetes
- Related autoimmune condition, e.g. rheumatoid arthritis, MS, Addison's
- Digestive problems, chronic fatigue, osteopenia/osteoporosis
- Down Syndrome
- Other related symptoms or conditions

- Number of persons in your family to be tested
- Their ages
- Why they should be tested
- Whether or not they are currently on a gluten-free diet
